Points to Note

  1. This form is only for enquiries relating to sexual harassment.
  2. This form is also available in Traditional Chinese, Simplified Chinese and eight other languages, such as Bahasa Indonesia, Hindi, Nepali, Punjabi, Tagalog, Thai, Vietnamese, and Urdu.

  7. The original copy of this enquiry form and any material provided shall be at the disposal of the EOC and will not normally be returned to the enquirer.
  8. Supporting information / documents can be uploaded where indicated in the form. (Up to 3 attachments, only files in PDF, JPEG, PNG and GIF formats can be accepted. The file size of each attachment should not exceed 4 MB). Further information / documents can be submitted by post or by fax separately, or when you are contacted by our staff.

Use of Personal Data

Any of your personal information that is collected or held by the EOC will be kept confidential, but the EOC may use such personal information for the purposes specified below:

  • handling and following up on your enquiries and complaints;
  • carrying out any of our statutory functions;
  • improving and monitoring of our services to meet the needs of our service users;
  • conducting research for service improvements;
  • notifying the EOC insurer in relation to a claim or defending a claim in which you are involved as a party;
  • sending you information and materials regarding our other services (subject to your consent);
  • complying with any laws, regulations or guidelines issued by regulatory or other authorities;
  • any other purposes agreed by you; and
  • purposes relating to any of the above.

If you have any questions or need assistance in completing the form, please call the EOC Anti-Sexual Harassment Hotline at 2106 2222. Upon receipt of this online form, you will be contacted by an EOC Anti-Sexual Harassment Unit officer. If you are yet to receive any message from us 14 working days after you send in your form, please call the above hotline as we may not have received your message due to technical issues.

    ^ If a complaint of the concerned sexual harassment case has already been lodged with the EOC, please contact the corresponding handling officer directly instead of submitting an online enquiry form.

    + Under anti-discrimination laws, if your complaint is made 12 months after the incident has taken place, the EOC may decide not to conduct, or to discontinue, an investigation into the complaint unless there are extenuating circumstances for the delay in making the complaint.
